INCIDENT REPORT

INCIDENT REPORT FORM

Please promptly report serious incidents or concerns to help us maintain the quality of our park. To report an incident, fill out the online Incident Report Form with all known relevant information. Use fact-based language and include the following:

 

  • Note the date and time when the incident occurred.

  • Specify the exact location within the park that the incident occurred.

  • Provide a detailed description of the incident or concern, including names of the owners and dogs involved, names of witnesses and all known information.

 

Your feedback is invaluable in ensuring the well-being of our furry friends and their owners.

 

Investigating the Incident

For confidentiality reasons, incident reports are sent only to the Friends of the Peekskill Dog Park Board. He/she then investigates the incident by:

 

  • Contacting the parties involved

  • Interviewing witnesses, if applicable

  • Reviewing camera footage, if available

 

After the investigation is complete, the President of the Friends of the Peekskill Dog Park provides a proposal about how to best resolve the incident. The FOTPDP board discusses this proposal, provides input and votes on whether to accept the proposal.  If approved, the President communicates the proposal to involved parties. If the board does not approve the proposal, the President revises the resolution as needed until board approval is achieved. The President presents this board-approved resolution to involved parties.

 

Corrective Actions

Based on the nature and seriousness of the incident, the President and FOTPDP board will consider a range of potential corrective actions. It's important to understand that these actions are determined on a case-by-case basis, considering all factors related to the incident. Potential measures may include, but are not limited to, the following options:

  • Restricting Park Access: If a dog injures another dog or person or continues to show aggressive behavior, park access may be restricted for a specific period.

​

  • Revoking Membership: Membership may be revoked if corrective action is not successful, and the dog continues to exhibit inappropriate behavior and/or if the conditions of re-entry are not met.

 

  • Immediate Termination: If a dog injures another dog or a person on two separate occasions, membership will be immediately terminated. If the owner has two or more dogs requiring corrective action, the owner may be banned from the park.
